To secure your in-person or phone appointment, please understand our request for a credit card number, as it discourages late cancellations.

For phone consultations, please have an email or fax available to ensure all necessary paperwork can be completed.

We ask that you please arrive 15 minutes prior to your in-person appointment. This ensures that you can complete any additional paperwork and still enjoy the full length of your schedules visit.
